New Athletes:
Download and print off the PHI for new athletes and transfer form. The first pages are to be read and stored with your other important documents.
After reading the information, fill out the personal health information (the last 4 pages).
Verify the forms are complete and accurate with the necessary signatures required for each consent and release.
Once verified, include an enlarged copy of the front and back of your insurance card (preferably on the same side of the page). Please note: If there is any background color on the card, you will need to copy it on the lightest copier setting.
Mail or fax all forms and the copy of your insurance card(s) to:
